For older adults, transitions in care are usually unplanned and can be disorienting. For those living with diabetes, these transitions are considered particularly high-risk with the potential for medication errors, inconsistencies in care, uncontrolled glucose levels and potential hypoglycemic events. These transitions often result in emergency department visits and hospital readmissons. Older adults and their caregivers can benefit from careful assessment and clear instruction on changes to dietary patterns, medication adjustments, and coordinated follow-up care. Registered Dietitian Nutritionists should be key members of the care transition team in all healthcare settings.
